RMS Health Insurance Group helps people with qualifying chronic conditions understand Chronic Condition Special Needs Plans (C-SNPs) and compare the plans available where they live. Call (412) 913-3669.
A Chronic Condition Special Needs Plan, or C-SNP, is a type of Medicare Advantage plan for people living with specific severe or disabling chronic conditions, such as diabetes, chronic heart failure or cardiovascular disorders. These plans tailor their provider networks, drug lists and care coordination to the condition they serve.
To join a C-SNP you must have Medicare, live in the plan's service area and have a qualifying condition, which is usually confirmed with your doctor. We explain how C-SNPs differ from other Medicare Advantage plans and help you see whether one fits your health needs.

People with Medicare who have a qualifying chronic condition that the plan is designed for, such as diabetes or chronic heart failure, and who live in the plan's service area.
How is a C-SNP different from a regular Medicare Advantage plan?
A C-SNP limits membership to people with specific chronic conditions and tailors its benefits, provider network and drug coverage to those conditions.
